Thanks for the report, Spoooky. This was a visual-only issue that could happen in a small window where the updated loot chest data was available but the patch that included the visuals for it wasn't yet downloaded. You should have actually received the 4th item in your collection even though it couldn't be displayed in the loot chest. We're working out ways to avoid this happening again in the future.
