There was previously code put into place to reduce the chance of this sort of thing happening. Going to pass this on to our Quality Assurance folks for investigation.

If anyone else runs into this, screenshots or a description of the area you experienced it in (like, looks like the Ice Caves or Act V Cemetary) is immensely helpful in tracking down any straggling issues. We'd love it if you'd post your reports in our Bug Report forum!
