“Design” typically refers to Game Design, which is more about core gameplay, modes, and reward systems. This was particularly confusing for me when I entered the game industry, because I identified as a “designer” my whole career.
“Visual Design” is the UI/UX of the in-game HUD and client (which is what I work on). This includes UI assets, typography, information architecture etc.
and “Art” is the term we generally use for the illustrations that are worked on in partnership with SMV. They are incredible and deserve all the praise they're getting!
Our art team and SMV have worked very, very closely on developing illustrations together. Also I don’t want to forget the work of design, narrative, tech art, engineer...etc, etc. my personal opinion is that one of the reasons the LoR Art is so good is because it feels so connected with the whole world and the mechanics of the game!
Oh and not all of the illustrations were made by SMV, you can actually look at the full screen art view to see which artist(s) worked on each and every piece of card art. I love seeing so many talented people in those credits!

A few confusing things I'll try and clear up.

Hope that helps.
