Diablo IV's Lead Class Designer, Adam Jackson, has confirmed some details on the 6 rarest unique items in the game - the super rare uniques. Originally, not much was known about these items, but as players have been slaughtering demons en masse, not only have they been found, but now we can confirm what they all are.

What are the 6 Rarest Uniques in Diablo IV?

The rarest uniques include two swords, two helms, a ring, and an amulet.

  • Doombringer - Unique Sword (Slashing)
  • The Grandfather - Unique Two-Handed Sword (Slashing)
  • Andariel's Visage - Unique Helm
  • Harlequin Crest - Unique Helm
  • Ring of Starless Skies - Unique Ring
  • Melted Heart of Selig - Unique Amulet

Here are their tooltips, as provided by Blizzard.

As you can see, each of these items will require level 65 to be equipped, but at least they are Account Bound so if they aren't useful on one character, we can use them elsewhere.

How to Obtain Diablo IV's Rarest Uniques

There isn't a specific way to obtain these rares, but there are a few things you need to keep in mind:

Quote From Adam Jackson

1. They can drop from level 85+ enemies
2. You get them anywhere you can get a regular Unique, and they always drop at 820 ipower
3. We currently have 6 of them in the game
4. They're realllly rare!

Adam went on to further clarify that these items have a chance to drop when you trigger a unique drop.

Quote From Adam Jackson

Whenever you get a Unique there is a chance for that Unique to instead be one of these. Therefore, the best way to farm these is to do content that gives you the most Uniques per x period of time.

All this information comes to use just a couple of weeks after Adam confirmed the items were in the game they are just "that rare".

Where Diablo IV's Super Rare Uniques Come From

These aren't just any items! Four of the super rare unique items refer to an items available in Diablo II!

  • Doombringer was a Unique Champion Sword. Huge damage!
  • The Grandfather was a Unique Colossus Blade.
  • Andariel's Visage was a Unique Demonhead and is named after Andariel, the Maiden of Anguish.
  • Harlequin Crest was a Unique Shako which provided a huge 50% boost to Magic Find.

Ring of Starless Skies & Melted Heart of Selig don't come from previous games in the series.

So there you have it. To get these items to drop all we need to do is defeat enemies that are at least level 85, and farm content that has the highest number of uniques dropping. The real question now is, who is going to get all 6 on their account in a season?
