Played Celticshark5's Face Auction deck (it's super cheap), but accidentally died because of fatigue since I drew too many cards. It seems that the deck will only refresh 4 times for a total of 30 cards, although it could be more - I didn't count. Keep this in mind!

The limit is likely per turn and not per match. Someone go verify it!
It might be if you start overdrawing while you have a full hand? Or if you just draw too many cards in a single turn as Flux suggests.
I was just playing the Mindblast OTK version listed on the site, and I over drew several cards, going up to 6 damage in a single fatigue draw. Luckily I was also able to play a couple Spirit Lashes and 4x Mind Blast to set myself up for a win and to survive 7 potential fatigue damage next turn.
However, as my next turn started, my deck reloaded. So it will fatigue if you try to draw forever in a single turn, but does seem to regenerate the following turn.
Interesting... Yeah, I wasn't sure how it worked exactly, but it's odd nevertheless.
I'll try to verify later today.
definitely not by drawing cause I made a draw heavy deck and got around 7 copies of a card.
Can confirm that milling cards will then result in fatigue damage. Was playing a discolock deck with three Malchezaar's Imp on board and playing Cataclysm killed both myself and my opponent. I milled the 6 cards and then took about 50 fatigue damage and my opponent got three Fist of Jaraxxus to their face.
So I played a few more games and I think I got it. If you overdraw the last card of your deck, it won't refresh. However, if you shuffle something into your deck (I swapped Claw with Jade Idol for the purpose of testing), the deck will refresh after you have drawn the last shuffled card.
